Registration
You'll need to fill out several important forms to help us keep track of your contact information and that of your teammates. The links below will take you to interactive versions that you can submit online.  You can also print the forms and fax them.   

NOTE: If you've already submitted your form and have changes or corrections, contact Kelly Williams at  859-299-2811.   

Form 1: Player's Fact Sheet  (Click here.)
This form must be completed by each participant in the tournament. We may occasionally have to contact players if there are last minute changes in schedules, etc.  Also, the information requested is important in determining handicaps, helps registration and check-in go smoothly, and assists WKYT-TV with their on-screen display of team names during their telecast.

Form 2: Liability Release Form 
This form needs to be signed by ALL players, celebrities, volunteers, participating children (in the Kids Jamboree, Larry Gilbert and other events) and anyone else who is involved in activities relating to any Children's Charity event.  Please print out and sign the Release Form. It must be faxed or delivered prior to participating.
