2010 Agency Applications for Funding

Attached below are the forms to be used in applying for funding from the Children’s Charity Fund of the Bluegrass, Inc.
Applications are due on February 26, 2010.

The Mission of the Children’s Charity Fund of the Bluegrass, Inc. is to establish and maintain an ongoing concerted
fund-raising effort to benefit local charitable organizations which have as their primary goal the betterment of children
of the Commonwealth. When allocations are made in June 2010 this will be the 29th year Children’s Charity has
provided funding to agencies in the Bluegrass area.

The Executive Board of Directors of the Children’s Charity Fund of the Bluegrass, Inc. has adopted the following
parameters in determining the allocations of funding.

1. We prefer to give directly to service organizations, not to a conduit organization.
2. We prefer to not fund salaries.
3. We will focus our contributions in the Central Kentucky area.

The following forms and documents must be sent by e-mail to Mollie Brennan at  Mollie_Brennan@ml.com for
the application to be deemed complete.

1. Form A - Application for Funding (MS Word / Acrobat PDF)
2. Form D - Projected Budget (Excel / Acrobat PDF)
3. Latest Financial Statements (Audit, Compilation or Review)
4. Board of Directors and Officers Roster
5. Also please include a success story from your agency (if you have been previously funded by CCC),
   one copy of the letter from the IRS concerning 501 (c) (3) status, and artwork for your agency's logo
   (in PDF, TIFF or high resolution JPG format).

The 2010 Tee Off reception will be held on Monday June 21:

4:00-4:30 Reception
4:30-5:30 Program and check presentations
