| 2011 Agency Applications for Funding
Attached below are the forms to be used in applying for funding from the Children’s Charity
Fund of the Bluegrass, Inc.
Applications are due on February 26, 2011.
The Mission of the Children’s Charity Fund of the Bluegrass, Inc. is to establish and maintain an ongoing concerted fund-raising effort to benefit local charitable organizations which have as their primary goal the betterment of children of the Commonwealth. When allocations are made in June 2011 this will be the 30th year Children’s Charity has provided funding to agencies in the Bluegrass area.
The Executive Board of Directors of the Children’s Charity Fund of the Bluegrass, Inc. has adopted the following parameters in determining the allocations of funding.
- We prefer to give directly to service organizations, not to a conduit organization.
- We prefer to not fund salaries.
- We will focus our contributions in the Central Kentucky area.
The following forms and documents must be sent by e-mail to Mollie Brennan at mollielbrennan@gmail.com for
the application to be deemed complete.
- 2011 Form A - Application for Funding (MS Word / Acrobat PDF)
- 2011 Form D - Projected Budget (Excel / Acrobat PDF)
- Latest Financial Statements (Audit, Compilation or Review)
- Prior CCC Funding, Program and Estimated Affedavit
- Board of Directors and Officers Roster
- Also please include a success story from your agency (if you have been previously funded by CCC),
one copy of the letter from the IRS concerning 501 (c) (3) status, and artwork for your agency's logo
(in PDF, TIFF or high resolution JPG format).
When providing Children’s Charity Golf Classic with the information above, please submit each item in a separate email with the following subject line:

- [Your Agency Name] Success Story
Request for a visit: Please send an invitation to a board meeting or an event of your choice to Mollie Brennan at mollielbrennan@gmail.com. This will help us in our due diligence efforts.
Request for volunteers: Ann Chapman Breeden, Volunteer Coordinator: Please be prepared to share up to 10 volunteers on either Friday or Saturday for the golf tournaments on June 25-26.
As you may be aware, Children’s Charity Fund of the Bluegrass, Inc. primarily raises funds from the Children’s Charity Celebrity Golf Classic held in June of each year. The events are as follows:
